Cancer by the Numbers: Cancer of the unknown primary

Cancer is named from the place it originates. This makes a difference on what type of treatments will be effective. Being diagnosed with cancer of the unknown primary (CUP) means that cancer has been found in the body but the place of origin is unknown.

If later, the place of origin is found then the type of cancer diagnosis changes from an unknown primary to the specific organ or body area that the disease first began.

Some cases of cancer of the unknown primary are unfortunately never found. Physicians will then decide on the best course of treatment based on two factors: The way the cells look under the microscope and which organs are currently involved.

Statistics and Prognosis:

The exact number of cases of this disease is not known. It may be about 2% to 5% of all cancers in the United States. It is found more often among men than among women. The average age of people with this cancer is about 60 years.

This is a very dangerous cancer. Only half of patients will live 9 to 12 months after their cancer is found. There are several reasons why this cancer is so serious. First, most of these are fast-spreading cancers. Second, because the exact type is not known, it is harder for doctors to know what treatment is best. Also, the cancer is often widespread, making it harder to cure.

In recent years, microscopic and other diagnostic techniques have improved dramatically. For this reason, doctors can now determine the primary site in about four out of five cases.

Thought for the Day: The facts on fish

Ever wonder what fish to eat, what fish to avoid, what fish is healthy, what fish is cancer-causing? I do.

I'm looking into this whole fish thing. And while my search for information is in no way exhaustive and my findings are far from conclusive, I have found some interesting fishy facts and figures.

Think about this:

Fish definitely has health benefits. It's low in fat, high in protein, and rich in omega-3 fatty acids. Americans love this. How do I know? Because on average, each of us eats a record 16.6 pounds of fish every year. Our intake of shrimp and salmon has doubled, in fact, since 1994.

Fish definitely has its drawbacks too. Headlines repeatedly warn us of dangerous contaminants in lakes, rivers, and oceans. Don't forget about mercury, the biggest fish health hazard. It's been linked to neurological problems in developing fetuses and children, making consumption of shark, swordfish, tilefish (aka golden snapper or golden bass), king mackeral, canned albacore tuna, and tuna steaks a no-no for hoards of women and children.

For just about everyone else, the benefits of eating moderate amounts of seafood greatly outweigh the risks. Just watch out for those PCBs (polychlorinated biphenyls) -- possible carcinogenic chemical compounds that end up in some seafood.

To avoid PCBs, steer clear of farmed salmon which contains high levels of these compounds or limit your intake to less than one single eight-ounce meal per month. Opt for the wild variety of salmon to avoid this concern altogether. Or take the side of the FDA. Their reports say salmon is a powerhouse when it comes to protecting heart and developing cancer from this source is much lower than the risk of heart disease.

Increased risk of bladder cancer associated with chlorinated water

A new study shows that if you drink, bathe or swim in chlorinated water it may increase the risk of bladder cancer. The chemicals, mostly chlorine, used to disinfect water can be harmful when they are inhaled or absorbed through the skin.

The chemical by-product produced, called trihalomethanes (THM), can be a powerful carcinogenic because it does not get detoxified by the liver.

The researchers analyzed the average water THM levels in the 123 municipalities in the study. People living in households with an average household water THM of more than 49 micrograms per liter had double the bladder cancer risk of those living in households where water THM concentration was below 8 micrograms per liter. They noted that levels of about 50 micrograms per liter are common in industrialized societies.

Those who drank chlorinate water were at a 35 percent greater risk of bladder cancer than those who didn't and the use of swimming pools boosted bladder cancer risk by 57 percent.

Something else we need to worry about--great! I am not losing sleep over this one. I cannot take back all those years swimming in the backyard pool!

Why do patients respond differently to standard doses of medication?

The standard dose of some medications are too high and dangerous for the patients, where some patients respond the exact opposite and show that the standard dose is too low to produce beneficial effects. It would seem to be a simple case of age, gender, or genetic differences to explain the individual variability in response to the drugs.

A study at the University of Kansas is reporting that variations in the body's production of hydrogen peroxide, which is believed to serve as a signaling molecule at low levels, can affect the accumulation of drugs inside our cells.

Oxidative stress, an increase in hydrogen peroxide levels, may have an increased response to a given dosage of a drug. This seems to show that it is in our best interest for physicians to provide more individualized dosing of drugs.

Hydrogen peroxide effects could be especially important in therapeutic drugs such as aminophylline, carbamazepine, lithium, carbonate, phenytoin, theophylline and warfarin. The researchers think that small changes in the doses of these drugs could cause either subtherapeutic or toxic results.

Scientists know how melanoma spreads

Researchers at the Marie Curie Institute have said that they now understand how melanoma spreads to other parts of the body. They feel that this information can help in uncovering how other cancers metastasize.

A gene called MITF controls a switching mechanism inside the malignant cell. It seems that MITF levels determine whether the cell is dividing, proliferating, forming a tumor, or whether it goes into a non-dividing state, which becomes highly invasive. The researchers were surprised and assumed that the dividing cells would become invasive.

Chemotherapy goes after dividing cells so researchers think that these non-dividing cells are the dangerous ones that lead to metastasis to other organs.

Mouth cancer insight opens doors for prevention, treatment

Scientists have determined that mouth cancer develops in two different ways which dictates the seriousness of the disease. This finding, revealed on Tuesday, could lead to better prevention and treatment. In laboratory experiments with healthy, early, and advanced cancer cells, researchers were able to pinpoint differences in the cells that determined the aggressiveness of the cancer. They found faults in the p53 gene, which stops damaged cells from dividing, and in the p16 gene, which helps regulate and prevent cancer from developing. Both changes are linked to more aggressive tumors. This is first-time evidence that some mouth cancer tumors are more aggressive than others and are unfortunately linked to poor patient survival.

Oral cancer typically stems from pre-cancerous lesions, changes, or patches in the mouth -- all of which are early signs of cancer. Recognizing which pre-cancerous changes are more likely to develop into aggressive tumors would allow doctors an insight that could help them prescribe the best treatment.

Smoking, use of chewing tobacco, and heavy alcohol consumption are the leading causes of mouth cancer. And smoking and drinking are a very dangerous combination. Like lung cancer, mouth cancer usually develops in people age 50 or older.
