Fear of skin cancer prompts call to action

I keep thinking about my ongoing negative relationship with the sun, how it burns me time and time again, how I keep trying to fine-tune my approach to dealing with this deadly force. Today, I have arrived at two new thoughts.

1. There was a time when I wanted a tan. I'd accept a burn even, in hopes it would turn to the slightest shade of brown on my pasty white skin. I would search high and low for the sun. I would drive in its direction, bask in its glory, give hours of my day to this crazy pursuit. Somehow, though, achieving a tan -- or burn -- wasn't easy. Sometimes, I'd see some color appear; sometimes my efforts seemed for nothing. It took work, effort, endless amounts of time and while my ventures in sunbathing did sometimes prove successful, there were many times I was left with, well, pasty white skin.

Fast forward to now. Not only do I seek shelter from the sun, but I use sunscreen, sit under umbrellas, and cover up whenever I can. Still, I get burned. It seems if I look in the direction of the sun, with my sunscreen-coated face, it will get burned. Long ago, my bare face only occasionally absorbed the sun. Why the change? Why when I worked not at all at protecting myself was it so hard to attract a golden glow? Why now do I protect myself in all ways possible and still sizzle? I'm wondering if it has anything to do with the chemotherapy drugs that poisoned my body for so long. A dermatologist once told me about a phenomenon called UV recall. The sun and the drugs can react, long after treatment has concluded, and can cause skin reactions. Maybe this is what's happening to me. Just in case, this gives me all the more reason to avoid all contact with the sun.

Thought for the Day: Headed for melanoma

Oh no. I think I 'm headed for melanoma. At the very least, I seem to have a very high risk for developing the disease, thanks to my once-stubborn pursuit of a silly tan.

Think about this:

A review of seven different studies concludes that using a tanning bed under the age of 35 -- I'm so guilty -- can increase the risk of melanoma by 75 percent. Even those who have ever used indoor tanning were 15 percent more likely to develop the disease.

We're talking the deadliest form of skin cancer here. So deadly some experts are recommending strong measures to restrict the use of tanning beds by young people. Adults should be discouraged from tanning, some say, but access should be limited for those under the age of 18.

New Jersey already has regulations in place -- those under 14 are banned from tanning salons and anyone between 14 and 18 must have parental consent.


If I could turn back time, I would listen to my grandma. She told me the sun -- and tanning beds too -- were no good. But I was young. And I didn't care.

Now I'm older. And I care. But it may be too late. It seems this could be one lesson I learn the hard way.

Sunshine in the forecast for skin cancer prevention

Way back in my sun worshipping days -- when I longed for a golden tan, logged countless hours scorching my body, and ignored my grandma's warnings that my pale skin was just not tough enough for the sun's powerful rays -- I would have basked in joy over headlines now surfacing in the media. They go something like this: the sun may actually fight skin cancer instead of causing it.

According to a team of scientists at the University of New Mexico's Cancer Research and Treatment Center, a little bit of ultraviolet B light is enough to stimulate a vitamin D immune response in the skin -- but it's not enough to boost skin cancer risk.

It's still true that sunlight is the main cause of skin cancer. But limiting exposure is the key to preventing the disease -- and for promoting bone health and perhaps preventing colon cancer too.

Researchers, whose findings are published in the March issue of
Nature Immunology, suggest staying out of the sun for now -- because they don't yet know what constitutes a little bit when it comes to UVB rays.

Unique characteristics of rapidly-growing melanoma

Not all melanomas are alike, and the characteristics of a fast-growing melanoma skin cancer begin in skin tumors that are thicker, symmetrical, elevated, have regular borders or have symptoms, according to the conclusions of a survey study completed by Peter MacCallum Cancer Center researchers. They also note that the faster-growing melanoma is more likely to affect men 70 years or older and individuals with fewer moles and freckles.

"In summary, this study provides descriptive data on the spectrum of melanoma rates of growth and insights into subgroups of patients with melanoma that are associated with rapid growth," stated the researchers. "We propose that this information on melanoma rate of growth be incorporated into education programs for patients and health professionals."

The advantage of identifying the more aggressive melanoma will allow for more aggressive treatment. Details of the study will be published in the December issue of Archives of Dermatology. Of the different skin cancers, melanoma is the most serious and most often appears as a change in size, shape, or color of a mole. A melanoma called Bruce

Strip, Search and Save spokesperson and former surfer girl Kathy Lette shared a story of how her melanoma came to be named Bruce in the Village Voice It's Time to get Naked feature as part of an effort to raise skin cancer awareness.

A self-confessed slathered in baby oil bake in the sun tanner, Lette told the reporter, "I would say my boyfriend used to get me to cut his name out of paper and sticky-tape it to my stomach so that I would get a tan tattoo in his name. The joke was, if I ever get cancer I'll have a melanoma called Bruce.''

It was a not-so-funny joke when Lette had skin cancer surgically removed from her neck earlier this year. It was then that she remembered, in a bit of irony, the suntan tattoo joke she had told years ago.

As a skin cancer survivor, Lette has joined the Cancer Council's Strip, Search and Save campaign. The premise of the campaign is that couples sans clothing and visually check each other's body for unusual changes. With a nudge and a wink, Lette said, "Not only is it important that they do it, but it could be fun," suggesting that this exam might add spice to the couple's sex lives.

Who said cancer awareness and prevention cannot be fun? Lette certainly is not taking the stodgy position that the attitude and approach need be dreary.

In addition to the Strip, Search and Save campaign, Puberty Blues author Lette's latest novel is called How To Kill Your Husband – and other handy household hints. Of course, as she says, if you are still fond of your spouse (and I would say most of us are quite fond of our spouse) have him join you in scanning each other's skin. Real sun tan in a bottle: plant extract changes skin pigmentation

Knowing the skin damage that results from prolonged sun exposure needed to achieve the golden tan, many fair-skinned people are staying out of the sun and turning to spray on or sunless lotion tans. These fake tans are skin dyes that do not change or darken skin pigmentation the way the sun might, but do give a temporary look of a glowing tan complexion. Advances have been made to these products so that the olden days of orange palms and streaky uneven lotion marks from sunless lotions are a mere memory, but still, it's not a real tan.

What if you could achieve a real tan, a tan where the skin pigmentation actually changes and darkens the same as it would if you spent hours in the sun, without spending hours in the sun exposing yourself to the premature aging, wrinkling and increased skin cancer risk damages of ultraviolet (UV) radiation?

In addition, what if this product, in actually darkening the pigmentation of your skin, provided protection from UV sun damage?

Although still in the experimental stages, scientists are working on just such a lotion, using a plant extract called forskolin, that prompts the pigmentation of the skin to darken. While the efforts are said to be focused on the treatment of individuals with medical pigmentation disorders -- if forskolin proves safe -- could a commercial product using the same compounds that safely promote a darkening of skin pigmentation -- a real tan -- and skin cancer prevention -- be far behind?

Sunscreen can protect or damage skin

Sunscreen, designed to protect the user from sun skin damage might actually cause sun skin damage if applied incorrectly and infrequently. University of California Riverside chemists examined the effects of sunscreen lotion on the skin and found that after a period of time, the lotion or cream is absorbed into deeper layers of the skin. If more sunscreen is not applied to block ultraviolet radiation (UV), the sunscreen that has been absorbed into the deeper layers of skin creates additional oxidative damage as UV rays are able to penetrate through to the deeper layers where the sunscreen has been absorbed. When sunscreen is applied often, this does not happen.

There is only one practical solution at this time and that is to apply sunscreen lotion or cream often, about every two hours or after sweating or swimming. Researchers suggest another potential solution would be for sunscreen makers to mix sunscreen with antioxidants because antioxidants have been shown to reduce UV-induced damage to the skin. Cancer prevention homework: study ABCDs of melanoma

I am an expert in the game of what-if. I guess it's because my recent what if this hard lump in my breast is cancer worry turned into Oh My God, it is cancer that I am so polished at this exercise in all things irrational. Sure, some worries will be fulfilled by reality but for the most part, things turn out okay. But still, I worry. When a bone hurt in my arm last year, I was sure it was bone cancer. It wasn't. When I felt a soft bump on the roof of my mouth, I whisked myself to the dentist for my mouth cancer diagnosis. It was just a little bit of inflammation, probably from a cold. A headache landed me in a tube for a scan of my head. It revealed nothing interesting, and ibuprofen fixed me right up. And lately, I am checking every mole, freckle, spot, speck, and discoloration that adorns my fair skin.

I asked about each of these what-if marks yesterday at my annual skin cancer screening. Surprise -- nothing is wrong with me. But all unfounded fears aside, there is some method to my madness about skin cancer because I have had several bad burns in my life, have spent too many hours in the sun in search of a tan, and have already had a few pre-cancerous spots removed from my skin. So I do plan to monitor my skin -- just maybe not every day -- for the ABCDs of melanoma. And you should too -- because melanoma is the deadliest of skin cancers, and it is known for spreading, which makes treatment essential. So consider these what-ifs when screening yourself for skin cancer.

What if a spot on my skin is Asymmetrical? This means that one half of the mark on your skin does not match the other half. See your dermatologist.

What if the Border of the spot is irregular? This means that the edges are ragged, notched, or blurred. See your dermatologist.

What if the Color of the spot is not uniform? This means that shades of tan, brown, and black are present. Dashes of red, white, and blue add to the mottled appearance. See your dermatologist.

What if the Diameter is more than 6 millimeters? This is about the size of a pencil eraser. Any growth of any mole should also be of concern. See your dermatologist.

It is estimated that 44,000 Americans will develop melanoma annually. Of these people, 7,300 will die. But the death rate is declining -- because patients are seeking help earlier. Perhaps the ABCDs of melanoma are helping. I hope they help you -- and me too.

Young girls shun the sun at higher cancer risk and bone loss

Between pale is the new tan and cultural or religious beliefs that require young girls to wear clothing that covers all skin areas and young girls who are restricted from outdoor activities, adolescent girls are paying the price in insufficient levels of vitamin D needed for peak bone mass -- increasing the risk of osteoporosis fracture later in life. By shunning the sun they might be decreasing the risks for skin cancer, but they are increasing the risks for 16 other cancers.

Of the 51 healthy girls tested, Saint Mary's Hospital for Women and Children in Manchester researchers found 73 percent were vitamin D deficient, and 17 percent were severely deficient in vitamin D.

Lack of a sufficient level of vitamin D was in direct correlation to the lack of exposure to sun and not to dietary intake. "This is in keeping with the fact that the main source of vitamin D is that produced by the action of solar ultraviolet B radiation acting on 7-dehydrocholesterol in skin," the team explains. "Only small amounts are obtained from dietary sources."

Aside from encouraging more outdoor activity for these young girls, the researchers stated that it remains to be determined how to meet the needs of older children and adolescents from cultures that avoid sunlight.

Vitamin D: lack of summer sun increases risk for 16 cancers

When it comes to reducing sun exposure cancer risks, the information all gets a little confusing and seemingly contradictory in nature. We are told repeatedly to stay out of the sun, as this is the single greatest risk to the development of skin cancer. Campaigns are launched all summer long about the benefit of shade, of avoiding the sun, and the media makes headlines out of it in an attempt to raise awareness.

So if we are all doing what we are supposed to in reducing skin cancer risks by avoiding the sun, what do we make of the new study supporting University of California research that solar ultraviolet B (UVB) radiation reduces the risk of 16 types of cancer? UVB rays are what promotes the body's manufacture of the essential vitamin D that acts as a prevention to cancer risk.

According to study authors Cedric Garland and William Grant, "Enhancing vitamin D status appears to be the single most important simple thing people can do to reduce their risk of cancer, apart from avoiding tobacco and moderation in the intake of alcohol."

It might be a matter of practical common sense that moderation is the key. In the summer, researchers estimate it only takes 20 minutes of skin exposure to the sun to achieve the optimum levels of vitamin D derived from UVB rays. For darker skin, it can require over an hour to reach the same benefit. After that amount of time, it has no additional benefit. In addition, vitamin D supplements can supply a person's daily need for vitamin D.

So shade, sunscreen and covering up are still the best bets in skin cancer prevention. However, without sun exposure, the body is at risk for low levels of vitamin D, and the lack of vitamin D is linked to an increase in 16 different cancers.

Drugs increase sun sensitivity skin cancer risk

If you wear sunscreen to protect yourself from the damaging rays of the sun that can lead to skin cancer, there are prescription drugs that can increase the skin's sensitivity to ultraviolet light leading to a sunburn even with the added protection of sunscreen lotion, according to a CBS News published article Some Skin Products Up Cancer Risk.

The drugs to be aware of are: diuretics, antibiotics, heart medications, diabetes medications, and nonsteroidal anti-inflammatory pain drugs. In addition, skin care products such as Retin-A, alpha hydroxy acid, microdermabrasion products and Balsam of Peru can put you at increased risk of sunburn.

"Depending on the type of drug and how long you've been taking it, it can remain in your system from one to five days," says Bill Bailey, RPh, director of specialized care centers for Medicine Shoppe in St. Louis. "To be on the safe side, always ask the pharmacist about the effects of your medications on sun exposure. They'll go online and get current, accurate information."

Staying out of sun saves lives, seeking sun steals lives

I wish I could reverse the damage I've already done to my skin after too much time spent in the scorching sun, in search of a tan. It's seems unfair that a tan is so temporary -- yet its damage is everlasting. And it seems crazy that so many people are still searching for a tan -- when it has become so clear that is it so harmful.

As many as 60,000 people a year die from too much sun -- mostly from malignant skin cancer -- according to the World Health Organization (WHO). About 48,000 deaths are caused by malignant melanomas, and 12,000 deaths are caused by other kinds of skin cancer. And 90 percent of these cancers are caused by ultraviolet light from the sun. Cancer is not the only side effect of sun exposure, though -- serious sunburn, wrinkling, eye cataracts, growths on the flesh of the eye, cold sores, and other illness can result from the sun.

We all need some sun -- the vitamin D that is produced in the body by the sun helps to prevent disease and immune disorders. But too much is dangerous and sometimes deadly. Yet almost all ill effects from the sun can be prevented. And the WHO has released a report that advises people to seek shade, to use sunscreen with an SPF of at least 15, and to stay out of tanning salons. The report operates under the premise that sunscreens be used not to prolong sun exposure but to protect the skin when exposure is unavoidable.

L'Oreal: unique new sunscreen offers greater skin protection

Sunscreen products containing Mexoryl SX or ecamsule, an ingredient designed to block the sun's harmful UVA rays, have been sold in Canada and Europe for over ten years, but have not been available here.

The Food and Drug Administration (FDA) has recently approved the over-the-counter sale of L'Oreal Anthelios SX sunscreen product containing ecamsule to consumers in the US.

While UVB rays are known to cause sunburn, UVA rays penetrate deeper into the skin and are associated with an increased risk for basal and squamous cell cancers and melanoma skin cancer. Anthelios SX is made by the French cosmetics company L'Oreal. Mexoryl SX is owned by L'Oreal, and is only available in L'Oreal products.

Sunscreens might provide some protection from the harmful rays of the sun, and should be used when going out in the sun. Still your best bet is to shade yourself when spending any length of time in the sun and staying out of the sun during the peak hours of 10 AM to 4 PM when sun can cause the most skin damage.

Confessions and regrets from a former sun worshiper

I confess. I was once a sun worshiper. I grew up in Ohio where a really sunny day was rare -- so on the occasion when the sun was bright and hot, I was in my back yard or at a swimming pool or at a lake soaking up the warmth and comfort of the rays that mostly burned my skin but gave me a glow that eventually turned the slightest shade of tan and made me feel healthy. It's ironic really -- that I felt healthy when the act of sunbathing is so completely damaging. And I knew this at the time and for the many years that followed -- and I still basked in the sun and vacationed in Florida and sometimes actually drove in the direction of the sun on a overcast day, in search of a tan that was never fully achieved because my skin is pale and fair and was never meant for any amount of sun exposure.

Aspiring to be Miss Maryland: perfect tan led to skin cancer

The perfect tan. Hours of each day devoted to the sun. Visits to tanning salons on cloudy days. For many sun worshippers, a golden tan is the currency of beauty. Currently Miss Tidewater, aspiring to win the coveted title of Miss Maryland, Brittany Lietz is a young woman who once aspired to having the perfect tan in the belief it would win her more beauty pageants. At only 20 years of age, her perfect tan turned into a diagnosis of skin cancer.

Lietz has been using her celebrity position as a beauty pageant winner as a platform for raising awareness of the dangers associated with too much sun exposure. She is speaking to high school students at their schools and manning booths at health fairs to warn others her age that the cost of a golden glow is too high a price to pay when you put yourself in harm's way. She has a white scar running across the back of her right rib cage, and she has had 20 other moles removed since she was first diagnosed with melanoma. These days, she uses self-tanning sprays to achieve the look of a tan because a tan is still an asset when you compete in beauty pageants. A pump-spray tan is as close as she wants to be to the sun.

"To me, being tan is not worth losing my life over," she said. "I'm going to be pale and that's who I am."
